Minuteman Senior Services

The covered entity (CE), Minuteman Senior Services, Inc., reported that an employee was subject to an email phishing scheme that affected the protected health information (PHI) of 11,330 individuals. The PHI involved included names, dates of birth, Social Security numbers, and medical information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on email security precautions and on the proper methods of identifying fraudulent email communications.
