Healthback Holdings, LLC

The covered entity (CE), Healthback Holdings, reported that multiple employees were the subjects of an email phishing scheme that compromised the electronic protected health information (ePHI) of 21,114 individuals. The ePHI involved included names, addresses, dates of birth, social security numbers, claims information, diagnoses, conditions, lab results, medications, and other treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE implemented additional technical safeguards and additional security training. During the course of the investigation, the CE’s business was sold.
