OCR description
The covered entity (CE), Eventus WholeHealth PLLC reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 1,707 individuals. The PHI involved includes names, addresses, zip codes, dates of birth, Social Security numbers, diagnoses/conditions, health insurance ID numbers, medical record numbers, and other treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE offered free credit monitoring to affected individuals.
