OCR description
The covered entity (CE), Sentara Williamsburg Regional Medical Center, reported that a manometry system that contained the protected health information (PHI) of 591 individuals was missing. The PHI involved included names, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative safeguards to better protected its PHI. OCR also provided technical assistance regarding the HIPAA Rules.
