OCR description
The covered entity (CE), Southwest Behavioral & Health Services, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 1,337 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and diagnoses. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. The CE also established a call center for questions or concerns. In response to the breach, the CE sanctioned the workforce member and strengthened its technical safeguards.
