OCR description
The covered entity (CE), Kareem Redmond, DDS dba Blue Wolf Dental, reported that an employee sent emails without using the blind carbon copy email function which compromised the protected health information (PHI) of 1,100 individuals. The PHI involved included names and email addresses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards and retrained the employee responsible for the breach. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule.
