OCR description
The covered entity (CE), OMNI Healthcare, Inc., reported that employees of an entity in which it had a contractual relationship accessed the protected health information (PHI) of 1,000 individuals. The PHI involved includes names, dates of birth, addresses, Social Security numbers, diagnostic and clinical information, lab results, and medications prescribed. OCR determined that the security incident did not constitute a breach.
