OCR description
The covered entity (CE), Medical Mutual of Ohio, reported that a vendor of its business associate (BA) experienced a ransomware attack affecting the protected health information (PHI) of 1,377 individuals. The PHI involved included names, addresses, dates of birth, and health information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the BA suspended its business relationship with the vendor.
