OCR description
Celanese Medical Plan, the covered entity, reported that an employee erroneously sent an email that contained the protected health information (PHI) of 704 individuals without using the blind copy function. The PHI involved included names and email addresses. The CE notified HHS and affected individuals. In response to the breach, the CE implemented additional technical safeguards.
