OCR description
The covered entity (CE), Farmville Internal Medicine, reported that it experienced a ransomware incident. The CE conducted a HIPAA breach risk assessment considering the four factors under 45 C.F.R. § 164.402 and determined that there was a low probability that its PHI was compromised as a result of the incident. The case was subsequently closed.
