OCR description
NYSARC Columbia County Chapter dba COARC, the covered entity (CE), reported that it experienced a ransomware attack that compromised the protected health information (PHI) of approximately 5,701 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and diagnoses. The CE notified HHS, affected individuals, the media, law enforcement officials, and provided substitute notice on its website. In its mitigation efforts, the CE deployed additional administrative, technical, and security safeguards. Staff were retrained.
