OCR description
The covered entity (CE), The State Psychiatric Hospitals in the New Jersey Department of Health, Division of Behavioral Health Services, reported that its business associate (BA) experienced a hacking attack affecting the protected health information (PHI) of 850 individuals. The PHI involved included names, dates of birth, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice. In response to the breach, the CE and BA implemented additional technical safeguards and provided complimentary credit monitoring services to affected individuals.
