OCR description
Trinity Health, the covered entity (CE), reported that it experienced a hacking attack that affected the protected health information (PHI) of 46,748 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, claims information, diagnoses and conditions, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards. OCR provided technical assistance.
