OCR description
The covered entity (CE), Walgreen, reported that one of its pharmacies was burglarized and a box containing the protected health information (PHI) of 1,704 individuals was stolen. The PHI involved included names, addresses, dates of birth, claims information, medications, and health plan information. The CE notified HHS, the affected individuals, the media, local law enforcement, and provided substitute notice.
