OCR description
The business associate (BA), Independent Living Systems, reported that it experienced a ransomware attack that impacted the protected health information (PHI) of 4,226,508 individuals. The PHI involved included names, dates of birth, addresses, Social Security and drivers’ license numbers, claims and financial information, diagnoses, conditions, and other treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach the BA strengthened its administrative, technical, and security safeguards. Staff were retrained.
