OCR description
The covered entity (CE), Dr. Alexander J. Richardson, DPM, reported that its business associate experienced a cybersecurity attack affecting the protected health information (PHI) of 11,651 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, and medications prescribed. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services to those affected.
