OCR description
The covered entity (CE), UCLA Health, reported that an unencrypted USB drive containing the protected health information (PHI) of 2,190 individuals was misplaced and not recovered. The PHI involved included names, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards, revised its policies and procedures, and retrained its workforce.
