OCR description
The covered entity (CE), Molina Healthcare, reported that its business associate (BA) mailed the protected health information (PHI) of 8,283 individuals to the wrong recipients. The PHI involved included names, member identification numbers, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and provided HIPAA-related training to its business associate. OCR provided the CE with technical assistance regarding the HIPAA Security Rule.
