OCR description
The covered entity (CE), Country Doctor Community Health Clinic, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 22,120 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, medical record numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services to affected individuals and implemented additional technical safeguards.
