OCR description
The covered entity (CE), Northern California Fertility Medical Center, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 12,145 individuals. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards, and updated its HIPAA policies and procedures.
