Cynthia Paul, M.D., LLC d/b/a The Coeur Group

The covered entity (CE), Cynthia Paul, MD dba The Coeur Group, reported that an employee experienced an email phishing incident that affected the protected health information (PHI) of 2,020 individuals. The PHI involved included names, Social Security numbers, financial information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE offered free credit monitoring services, established a toll-free number for questions or concerns, and implemented additional administrative and technical safeguards to better protect its PHI. Staff were retrained in email security precautions.
