OCR description
The covered entity (CE), Anesthesia Services of San Joaquin, reported that its business associate (BA) experienced a ransomware attack that affected the protected health information (PHI) of 43,924 CE patients. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, claims and financial information, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the BA strengthened its administrative, technical, and security safeguards.
