OCR description
The covered entity (CE), Providence WA Anesthesia Services, reported that its business associate experienced a ransomware attack affecting the protected health information (PHI) of 98,414 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, medications prescribed, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule.
