OCR description
The covered entity (CE), Center for a Brighter Tomorrow, reported that an employee lost an unencrypted USB drive that contained the protected health information (PHI) of 1,082 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, medications prescribed, and claims information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE reminded disciplined the employee involved and implemented additional technical safeguards.
