OCR description
The covered entity (CE), Extended MLTC, reported that a vendor of its business associate experienced a hacking incident that compromised the protected health information (PHI) of 5,494 individuals. The PHI involved included names and addresses. The CE notified HHS. The CE’s assets were purchased and it is no longer in operation.
