OCR description
The business associate (BA), Reid and Riege, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 610 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, medications, and other treatment information. The BA notified HHS and the media; the CE provided individual notification. In response to the breach, the BA implemented additional administrative and technical safeguards and retrained its staff.
