OCR description
The business associate (BA), Zomo Health, reported that the protected health information (PHI) of 1,359 individuals was inadvertently made accessible via its website. The PHI involved included names, dates of birth, Social Security numbers, addresses, health insurance information, phone numbers, and email addresses. The BA notified HHS, affected individuals, and provided substitute notice on its website. In response to the breach, the BA strengthened its administrative and technical safeguards and provided complimentary credit monitoring services to affected individuals.
