OCR description
The covered entity (CE), Delta Dental of Washington, reported that its business associate (BA) was the subject of a ransomware attack that affected the protected health information (PHI) of 6,361 individuals. The PHI involved included names and addresses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and ended its business relationship with the BA.
