OCR description
The covered entity (CE), Sage Counseling Omaha, reported that an employee impermissibly downloaded the protected health information (PHI) of 1,359 individuals onto a USB drive. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, claims and financial information, diagnoses, medications, and other treatment information. During OCR’s investigation, the CE ceased operations and the investigation was therefore closed.
