Premier Physical Therapy and Sports Performance, the covered entity (CE), reported that two employees were the subjects of an email phishing scheme that affected the protected health information (PHI) of 982 individuals. The PHI involved included patient names, dates of birth, addresses, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data and conducted additional phishing awareness training.
