OCR description
The covered entity (CE), The Valley Hospital, reported that the protected health information (PHI) of 4,245 individuals was improperly disposed of in a standard recycling ben without being shredded. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and physical safeguards to better protect sensitive data.
