OCR description
The covered entity (CE), Casco Bay EyeCare, reported that a USB drive containing the protected health information (PHI) of 1,432 individuals was lost. The PHI involved included names, dates of birth, and diagnoses. Subsequently, the USB drive was found and OCR closed the case.
