OCR description
The covered entity (CE), HH/Killeen Health System dba Seton Medical Center Harker Heights, reported that several employees of its business associate (BA) were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 15,056 individuals. The PHI involved included names, dates of birth, and treatment information. The CE notified HHS, affected individuals, and the media. In its mediation efforts, the CE and BA provided complimentary credit monitoring services and strengthened its technical safeguards.
