OCR description
The covered entity (CE), Springfield Clinic, reported that it misplaced a USB flash drive containing the protected health information (PHI) of 802 individuals. The PHI involved included names, dates of birth, and addresses. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained workforce members to better protect and secure PHI.
