OCR description
The covered entity (CE), Blue Cross and Blue Shield of Kansas, reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 1,308 individuals. The PHI involved included names, financial and health insurance information, and treatment information. The BA notified HHS and the affected individuals; the CE notified the media and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals.
