OCR description
Dr. Alain A. Montiel, DDS, the covered entity (CE), reported that a laptop was stolen from his vehicle that contained the protected health information (PHI) of 17,157 individuals. The PHI involved included names, diagnoses, lab results, medications, and other treatment information. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE implemented additional administrative and physical safeguards to protect PHI.
