OCR description
The covered entity (CE), Friend Family Health Center, reported that an employee was the target of an email phishing incident that affected the protected health information (PHI) of 1,419 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, financial information, diagnoses/conditions, and other treatment information. In response to the breach the CE implemented additional technical safeguards.
