OCR description
The covered entity (CE), Dr. Scott B. Klimaj, DMD, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 1,558 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information and medical information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE offered free credit monitoring to affected individuals and implemented additional administrative, technical, and security safeguards. Staff were also retrained on obligations to protect and secure PHI.
