Phoenix Programs of Florida, the covered entity (CE), reported that employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 6,547 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, passport numbers, financial information, health insurance information, and clinical information. The CE notified HHS, the affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE retrained its workforce and strengthened its technical safeguards.
