OCR description
The covered entity (CE), Massengale Eye Care, reported that its business associate (BA) was the victim of a cybersecurity incident that compromised the protected health information (PHI) of 15,000 individuals. The CE notified HHS, affected individuals, and the media. The PHI involved included names, addresses, dates of birth, and Social Security numbers. OCR has initiated a compliance review of the business associate regarding this breach.
