Mid-Westchester Anesthesia Services PC

The covered entity (CE), Mid-Westchester Anesthesia Services, reported that its business associate (BA) was the victim of a ransomware attack affecting the protected health information (PHI) of 871 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, health insurance information, medications prescribed, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional technical safeguards.
