Forest Hill Pediatrics, LLC

The covered entity (CE), Forest Hill Pediatrics, reported that its business associate (BA) experienced a hacking attack affecting the protected health information (PHI) of approximate 4,850 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license and Social Security numbers, email addresses, health insurance and claims information, and other treatment information. The CE notified HHS and posted substitute notice on its website. The BA notified affected individuals and the media. In response to the breach, the BA offered complimentary credit monitoring services and established a call center for questions and concerns. The BA also strengthened its physical and technical safeguards.
