OCR description
The covered entity (CE), Coastal Horizons Center, reported that an employee mistakenly emailed the protected health information (PHI) of 679 individuals to its business associate. The PHI involved included names and treatment information. The CE notified HHS and the affected individuals. In response to the breach the CE sanctioned the employee responsible and implemented additional administrative safeguards.
