The covered entity (CE), the State of New Jersey Department of Human Services, Division of Medical Assistance and Health Services, reported that an employee impermissibly sent the protected health information (PHI) of 3,900 individuals to an individual who was not authorized to receive it. The PHI involved included names, dates of death, Social Security numbers, health insurance information, and other identifying information. The CE notified HHS and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect PHI.
