OCR description
The covered entity (CE), Sacramento County Adult Correctional Health, reported that its business associate (BA) inadvertently made the protected health information (PHI) of 5,372 individuals viewable over the Internet. The PHI involved included names, dates of birth, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the BA implemented additional administrative and technical safeguards to better protect sensitive data.
