OCR description
The covered entity (CE), Assurance Health System reported that several employees were the subjects of an email phishing attack that affected the protected health information (PHI) of 3,565 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, medical record numbers, diagnoses, medications, and other treatment and health insurance information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented multifactor authentication as well as other measures to strengthen the security of its network.
