Restorative Arts Physical Therapy, Inc. d/b/a Core Conditioning

The covered entity (CE), Restorative Arts Physical Therapy dba Core Conditioning, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 727 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, medications, and diagnoses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
