OCR description
The covered entity (CE), Mohave County Adult Detention Facility, reported that an employee of its business associate (BA) inadvertently posted the protected health information (PHI) of 558 individuals to the Internet. The PHI involved included names, dates of birth, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative and technical safeguards and provided complimentary credit monitoring services to affected individuals.
