OCR description
The covered entity (CE), Heart of Florida Health Center, reported that an employee of its business associate (BA) exposed the protected health information (PHI) of 721 individuals over the Internet. The PHI involved included names, Social Security numbers, medications, medical diagnoses, and claims and other treatment information. The CE had notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative and technical safeguards.
