OCR description
The covered entity (CE), Titan Health Partners, dba Astera Cancer Care, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 3,848 individuals. The PHI involved included names, diagnoses, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical safeguards and trained workforce members on email security.
